Patient Doctor Agreements:

The purpose of these agreements is to allow us to more completely serve you and to get the best results in the shortest amount of time.  It is our experience that those patients who adhere to the following agreements get the best results.
 

Signing In:

When you arrive, sign in, you will be called on and assigned a treatment room in the order of your appointment time.
 

Payment of Bills:

We will expect you to honor the financial agreement you have made with our office.  If you find that you cannot fulfill the agreement you made with us, advise our staff immediately so new arrangements can be made.

Cancelling or Changing Appointments:

We have setup a specific course of treatment for you, to obtain the maximum results in the least amount of time.  Thus, if you need to change the time of your appointment, plan to come another time in the same day.  If the same day is not possible, be sure to make up the missed appointment within the same week.
 

Progress Evaluations and Re-Examinations:

During your treatment series, progress evaluations and re-examinations will take place to determine the amount of progress you have made.

Nutritional Supplements and Supports:

Nutritional supplements should be taken and supports worn as recommended by your doctor.  We do not prescribe, but will make recommendations to help speed up your recovery.
 

Auto Accident:

Patient Payment:

Patient involved in an auto accident receive 100% coverage under insurance in most cases.  The only patient out-of-pocket expense would be for food supplements, or any fees the insurance refuses to pay.

Explanation of Benefits:

Auto insurance covers your health expenses resulting from an accident as long as the insurance company has been notified of the accident in a reasonable amount of time.  Your insurance rates should not be afflicted by the cost of health expenses.

Office Policy:

Patient must notify insurance company that they are being treated at our office and have forms sent directly to us.  Ins the event the auto insurance denies that you hold insurance or refuses payment, that portion will be transferred to you and you will be responsible for payment.

If a lawsuit is involved with the accident, a lien form will be signed by you and your attorney ensuring us that we will be paid at the time of any settlement.  You are responsible for any balance over the settlement amount.

Please notify our office of the name of y our attorney or ask us for names of attorney's in your area.

Monday, Wednesday, and Friday: 8:00 to 11:30 and 2:00 to 5:30

Tuesday Closed

Thursday 9:00 to 12:00

Saturday 9:00 to 11:30

After hours appointments are available on an emergency basis only.
